Add captcha to existing form

I need a captcha placed on my attached html page. I have also attached the php form handler script.
I need the the captcha to be easily read by site visitors with clear letters, but not so easy for bots.? I would prefer to **NOT** use Re-Captcha or any other captcha service which requires dependencies on a third parties captcha servers. My hosting company? has the? GD library enabled? and working? as well as? Freetype support, so ideally this is what the solution should utilize.
The chosen coder will have to mount the working form on a website for me to view and approve, they must also give me the files source files. The source files delivered **must** be easy to edit so that this form can be used on other websites.

I re-read my project brief, and though it's not explicity stated (though certainly implied), I wanted to make sure that it is clear to all who submit a bid that the final solution MUST incorporate captcha along with the form validations that are a part of my existing form handling script (the one I attached to this project).
