Staff are invited using a URL including randomly generated token, to a feedback web page, the token lasts 36 hours.
A database holds Staff ID (25 char), name (40 char), department (40 char) & email address (80 char), and number of days since last survey (integer)
Random token is 5 characters. URL example; [login to view URL]
There are 6 static questions.
Feedback manager can login & see questions & scores, using filters, by date, by department
Manager adds the staff details.
Manager controls when feedback invites are sent, by displaying all staff, filtered by department, sorted by last survey, selecting a tick box to send, which generates a URL plus random token (check it's not already in use)
Output to screen is a list of Staff ID, Name, Department, email address, URL (inc token)
A database will record the customer ID, date & time survey accessed, network IP, and scores between 1 - 10 for each question.
Staff can also add a comment (250 char).
If feedback left before, the last 3 x 'dates & times, scores & comments' are also displayed as new feedback is offered.
When feedback is complete, Staff asked if they would recommend (yes / no)
A final submit button completes the feedback, cancels the token and emails an alert to manager.

I have gone through your requirement and have understood the requirement. I will be using python and host it free of cost on Google App Engine.
Below are the implementation details for your requirement:
1) There will be 3 kind of users: a) admin b)manager c)staff
2) Admin can add any number of managers and various department
3) Managers can add any number of staffs
4) Different dashboard for admins,managers and staffs
5) List of all the surveys initiated by the manager
6) The surveys can be exported as excel sheet. The excel sheet can also be exported on the basis of the filters used.
7) Once the manager initiates the survey, the staff will get the url in his email.
8) Once the staff completes the survey, the survey will come in the manager's dashboard and the token will be deleted.
