Add "Check Expiration Date" on 4Images Gallery

In need of the PHP code that would query my MySQL database, allowing members of my 4images gallery to check their membership expiration dates. My goal would be to have a submission box that members can enter their member name into and submit, which would then bring back their date of expiration. If I can get the functional PHP code, I can do all of the html necessary to then integrate it into my website. I am using v1.7.1 of 4Images
