Collect user information form

I require a php developer to write a php script for an article in joomla that links to the user table to collect additional user information. I will provide the database design ERD. I also have the data for the country, state and city drop down lists, so there is no requirement to provide any data. You will need to create your own sample data, a few records for testing purposes. The form must collect about 50 fields of data per user and it must write the data back to the database using the joomla sql framework. See sample code below. The form will mostly collect information from populated drop down lists that will trigger additional drop down lists up to 3 levels. Some check boxes will be required as well.
