Chronoforms PHP Javascript

I have an existingform that is php based with javascript included for calculations. I need this form converted to work with Chronoforms 4.0 RC2.0

Please note the pre-filled (0.00) fields towards the bottom of the form, "Total amount requested in this grant" must equal the sum of the fields below in the "Grant Request Details" section.

The new site is running Joomla 1.6
