CSS expert for customising my site.

Hi. I have built a site using a wordpress theme I purchased. There are three areas that I need custom work done: 1. I have a simple javascript quiz - users answer ten questions and depending on their score, they are directed to an answer page. The script works fine however I need the following done: - the radio and submit buttons need to be styled in the same CSS of my site. - instead of each question appearing underneath each other (there are 10 questions), I would like one question to appear and then a "next" button to allow the next question to appear. - the client's answers need to be emailed to me. 2. HTML Form development. I have a large HTML form covering about 15 questions - some multiple choice and other free text. I do not want the form to be one long lost of questions, but rather broken up into five sections so that it does not appear so long to complete. Alternatively, I am wondering if it is possible to have the one form split across different tabs on my page. Again, the CSS of this form needs to match the rest of my site and is possible a progress bar would be good. I have attached two screenshots of relevant pages of the site as it exists now.
