AutoFill Form Values

I am looking for help with a project I am working on. I have a PHP website for a sports league that users will fill out a form that submits 4 fields (Quarterback, Running Back, Receiver, and Kicker). Four each field I need to have two drop down menus (Team, Player).

The idea is that a user would select the team first in the Quarterback field, for instance, after the team is selected then it would automatically fill the Player field with the appropriate players in that team.

Then they would go down to the next line and do the same for each of the four fields that is submitted and then submit the form. Information submitted will be stored in a MySQL database.

The problem I have is that I need both the team fields and the player fields to be pulled from a MySQL database as the teams and players would need to be updated occasionally.

I would like the code to be somewhat simple to understand, I do have some experience in PHP, MySQL, HTML, etc as I would like to understand how it works and possibly modify it in the future.

Code must be Open Source so that I can see what it does

Installation instructions and any other special instructions must be included

I do have the Database structure currently built and can send a .sql file to the selected coder.

There will be no access given to the coder to my server. Code must be built on coders server and then I will install and test it prior to releasing payment.
