Build a simple data-driven web application in PHP [internal-01]

I need to create a simple application using PHP and a sqlite database. The application will run under Apache on a Linux server. The application is a simple tool to track background information about software developer contractors. It does not need to be beautiful. I will provide the CREATE TABLE statements that define the basic data that the application captures.
The application will have the following tables:
* Contractor info (name, address, phone, email, etc., comments)
* Skill lookup table (name of skill)
* Contractor-skill table (name of contractor, name of skill, and name of
* Project History (date, comments)
I will provide black-and-white sketches of what the screens look like. I don't care if the style is ugly. It doesn't matter if it looks good on a mobile phone or not. It only needs to work well in Google Chrome.
The application will need screens to do the following things:
* Add/Update/Delete a contractor record
* Add/Update/Delete contractor-skills records
* Add/Update/Delete a project history record
* See a contractor overview (display the base info, skills, and work history all on one page)
* Login (application will have just one password, set in a configuration file)
* Search for a contractor
The business rules and field validation will be very simple and limited.
