We would like a system, programmed in PHP, utilizing PEAR functionality (with postgreSQL or mySql as the backend) that allows users to 1) Log In 2) Submit webforms (your choice: html or PDF) 3) Have answers stored in a database (across several tables) 4) Have said webforms populate a pdf template to be emailed to one or more individuals (please describe method and 3rd party licensing costs, if any). Save for the PDF generation this is basically a CMS, though no one but the creator of a record should be able to read it. There should be three levels of access. The first, anonymous, allows anyone to complete a form. The second is available after registration, and provides registered users to edit records created by them. The third is administrative and allows a very small number of users to alter any aspect of any record (including registration information) The forms, scripts and output should use SSL, and Logins should have some protection against cookie spoofing. If you have any further questions please let us know. As I said this is for the most part a standard CMS and any apparent complexity is likely due to poor explanation on my part. A more detailed (and hopefully more helpful) outline is attached.
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
