ASP Vacation Survey Request

Hi All, I have a small company website that I need to do a vacation shutdown survey. I envision a web page that has a 12 month calendar that an employee can choose 3 different weeks that he would like his vacation to be on. Then there would be an adminstrator page where results would be by the week with percentages. I.E. Week 1-Jan 1-7- 1.8% Week 2-Jan 8-14- 21.2% Week 3-Jan 15-21-13.6% Etc. Perhaps also having a bar chart with the different weeks charted. This site is hosted on an NT Server and must be ASP driven, with an Access Database.
