CSS Design - Simple Form

I have a series of forms that will be generated from an internal application. I need them all to have a similar style. They must be a) email friendly, b) printer friendly, c) fax friendly and d) html / screen friendly. I'm going to embed a QR code in the header, so it'll need to have enough margins or whitespace around it to be readable by a QR reader. The template I started with came from an [login to view URL] order confirmation email. It was self-contained HTML and was visually very nice. I've started to customize it, but I need someone to help me get it to the finish line. It's not lined up the CSS could use a major cleanup. I'll provide an HTML file of my progress so far. One version will need a Terms & Conditions paragraph and a signature line. The second version will need a special area for marketing promotions / specials and whatnot where we can feature specials and promotions.
