Create Survey for Mobile Browsers

Hi

A simple and quick project.

I have attached a questionnaire which needs to be coded for mobile browsers.

Requirements

1) 1 question per page

2) All questions are required. Please add form validation.

3) Should be compatible for all types of mobile browsers

4) Centralized stylesheets required

5) Use javascript.

You should be able to host on your server and then share link to survey.

We will test by entering the link in a mobile browser.

This is a quick project and it will be great if you can convey the turnaround time.
