Convert ASP/Access application to PHP/mySQL

I need someone with experience in:

- PHP/mySQL

- Wordpress

This application stores its data in an MS Access database. I can send you the database file.

I need this ported to PHP/mySQL, and it will be used as a section on an existing Wordpress site.

I need a system that has all the same functions as the ASP version. This includes, but is not limited to:

Ratings must be approved by me before they are posted, and I must have the ability to edit the ratings. Ratings are made by visitors who must check a box saying they have actually had a claim involving the entity rated.

You can make this as a plugin accessed by the backend of Wordpress, or you can make it have its own separate backend. Being accessible from WP backend would be a plus.

As you can see in the application, ratings are in 5 categories: adjusters, body shops, contractors, attorneys and insurers. The visitor must give the location of the entity rated (city, state and country) and they will be in alphabetical order. The ratings must also be "searchable". So for example a visitor could see a list of all the adjusters for State Farm Insurance in Nashville, Tennessee, for the purpose of finding one with a good rating to handle the visitor's claim

As for the design, you can use the stylesheet/theme of my existing Wordpress site. The front end of the application must be incorporated into the Wordpress blog, it can be on a static page there.

If you know of any existing Wordpress plugins that will help with this task, I am open to building on those.

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):

a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
