Ajax / Javascript Fix - VB.NET, ASP.NET

Need a control I wrote cleaned up and tweaked or you could write it from scratch. The control is running here: <[login to view URL]> it is more or less doing what I need, but it needs some tweaking, testing and cleanup. There are 4 of the controls on this page, it is using the AjaxToolkitModalPopup extender, I would prefer this be switched to a standard javascript showModalWindow, so only 1 instance would be required or none at all, however more than 1 button? will need to? launch it, so I will need example code to get back the value and populate? a text box with the value of the selected file. I would also like the upload functionality added preferably with progress indicator and again I would prefer for this control to be a popup so only 1 instance needs to be on a page or non at all via a modal window and return value as described above. Hopefully you will understand what I am looking for.
