Create PDF to editable Google Doc

I am an attorney who needs to fill out a California Department of Motor Vehicles Form SR-1 for my clients. You can find this form in PDF format by googling it. I've also included a link below and attached it to this post. This form requires me to input a lot of basic information about my clients like their name, insurance info, date of birth ect. My goal is to have my SalesForce practice management software auto-populate these fields, but SalesForce can only auto populate a Google Document, not a PDF. That is why I need this PDF somehow converted into a fully editable Google Document. If done correctly, I will be able to enter certain codes into the new Google Doc (i.e., [Client_Name]) into the text boxes, and then my software can auto populate the document for me.
