App Store Application - Survey with branching logic and interconnected design

Create a mobile application to be hosted on iTunes consisting of a home screen, survey, results screen, results log, and information pages. The survey consists of 450 questions with built in branching logic based off of the user’s response to each question. Survey questions will take on various forms including multiple choice, simple data entry, open field text responses, image selection, etc. A specific question format used in the survey will be an “active image”: the image will contain multiple parts that the user can choose from. The image will light up when the user selects a specific part, and the user can select multiple parts on the image. Each question will have weighted answers that contribute to an algorithm that outputs an overall score. Question answers will also be weighted according to previous question answers to create an interconnected design. The application will also passively acquire outside data such as weather, location, and time and link this information into the algorithm. The overall output score will be presented on a results screen. The overall score can then be logged in the results log for the user to easily refer back to. In addition to the overall score, the application will generate a written summary based on specific individual answers to designated questions. This summary will be stored in the results log as well and can also be exported or printed out. The user will also be given the option to read more information that supplements their overall score result. This information will be housed in various information pages within the application. Other responsibilities will include: i) support for program debugging and reprogramming for a pre-released version of the mobile application with beta testing, ii) release of the fully operational mobile application on iTunes with the release of all associated code, and iii) technical support for 1 month post application release (additional funds may be provided for technical support extending beyond this timeframe). Our team will provide the wireframes for the design of the application. All artwork will be provided by our group to the programming team, including all hex codes, icons, and 2X and 3X image sizes.
