Write an iPhone and Android Inquiry application

Create a mobile application for an inquiry form in android and IOS with similar fields as here : [login to view URL] with the same logic (whenever user clicks on other in the dropdown, the field to specify the type appears). I also need login and registration functionality, the user has to register once and verify his email the first time when he opens the application. The registration will have the following fields : 1. Shop Name 2. Branch 3. Email After registration and email confirmation, the user will just have to log in once and should remain logged in every time he uses the app, the shop name and branch should automatically be entered, the rest of the form remains with similar fields as in the link above. After the form is submitted, it has to send an email with the corresponding fields. I do not mind the use of IONIC or core language, you are free to use whatever framework you are comfortable with, make sure you keep the bid price low.
