Coding ONLY - iPad Database App

I have the UI for an iPad app but just need the coding completed. It will only be about 5 screens, each screen a series of fields/forms that collect information (peoples details when meeting them for the first time). Think of it as an address book. In a 30 minute period i'd expect to gather 10-100 people's details, at the end of the day potentially 500+, and I wouldn't always be connected to the internet so I need these details to be saved locally within the app for a mass upload at the end of the day (either by emailing a csv file, or by plugging the iPad in to the computer). As the design would be done by myself, and i'd be using the app myself I'm looking for a simple solution. Animations a plus but doesn't have to be fancy, looking for function and speed at this point.
