I have a word document which have questions.
For example
First Name: text box
Surname: text box
Address: textbox
Have driving license: checkbox yes/no
I would like to have an android app with all these questions, including taking signature at the end,
The results or answers can then be either uploaded to an ftp or send to an email address in form of either csv file.
Also need to take photo of person and photo of proof passport and upload with the csv.
