Android Evo Shift 4G Wimax Center Frequency App

Required is a simple App that allows the user to change the WiMAX Center Frequency setting of an HTC Evo Shift 4G, and simple English documentation. The app should (a) display the current Center Frequency values, (b) have 3 separate fields for the user to enter the new values, (c) an "Okay" button to accept the changes, and (d) a "Cancel" button to ignore the user entry and exit the app. The end result must be that the unit's Sequans SQN1210 chip is able to scan on the frequencies entered by the user. It's okay if the app requires root access. I will own all rights to the app and source code upon completion.
