Deign an Access database to track repairs

Daily a file will be imported to Access. The file is the attached document "Quality Report(sample)". The columns that have been highlighted will be used in the access database. The form layout is per file "SR Database Layout". The fields in black will be loaded from the imported file. Once an individual selects the "Request Num" the other black fields will be populated. The technician will add data to the fields in red. Two dropdowns that are red will be associated with two different tables. The fields in the orange box will only be active if the FAR Required box is checked.
