Dental Tooth Chart - Web Interface

I need a tooth chart graphic/user interface for a web app I am building. It should have two main features, users should be able to use it to select individual teeth and there should be a javascript action called when this happens and secondly the teeth surfaces should be able to be coloured to signify work that has been carried out on those areas ie the teeth should be able to be overlaid with a transparent coloured layer (an example is shown in the open dental chart below). The surfaces able to be colored should be the Buccal, Lingual, Incisal(for anterior teeth) and Occlusal(for posterior teeth), mesial and distal surfaces. Information about the positioning of these is easy to find on google and as I am a dental student any questions can be addressed to myself also. Additionally it should be possible to hide teeth from view (in the case that they have been extracted), ideally it should be possible to colour the root canal or to replace the root with a screw to signify an implant or to hide the root entirely with out hiding the crown of the tooth. Finally the technique used does not bother me too much as long as it is inline with web standards and doesn't use Flash or another plugin. I think this would leave HTML5 or SVG. As long as I can manipulate it using javascript I am happy. Also I would like it well designed in color similar to the opendental diagram. It would be good to have lines like included in the Tooth chart medium file as well. It needs to be able to autosave to sharepoint or dynamics CRM notes. (There is a lot of ways to accomplish this) Lastly, for extra credit if you can make it have existing conditions, treatment performed, and needed treatment, and have the treatment performed auto populate a list as well as needed treatment, then that is even better.
