Create forms in HTML for Frontpage website use

I have website that is up and is written in old program called Frontpage. Host changed servers so my forms that I use for my business no longer can be sent to me because they no longer offer frontpage 'extensions'. Work around is to build the forms I need in HTML and uploading them to my site. There has to be code in there that sends the forms to my email when it is submitted, and give them a thank you page also, with a link built in back to the home page of my website. Can you help? Wouldn't hurt if key info such as applicant's name, addresss, email, phone number, etc. could automatically be captured in excel to preserve automatically.
