I have a "Paid Surveys" domain that is suspended in my Google AdWords account. I need a sqeeze page that should include a form with followig fields: Email, First Name, Last Name. The page should comply Google AdWords Policy. I will release the milestone only after the website will be unsuspended. I can give you my current html page and you can modify it.
