Create PDF Forms

Hi, I need someone to take the PDF's attached which were the files created for the printed version of this client's invoices, and turn them into interactive PDF forms which can be typed into and ticked where required. With the invoice numbering, the number that is in pink on the PDF's attached actually needs to be incremental so that each time the invoice gets opened it uses the next number. The 2 forms are just duplicate copies with one small difference in the invoice number system at the top. This needs to be done fairly quickly. I can supply the original InDesign files if required.
