convert video to flash banner with a download form at the end

I need to convert the attached video in a flash banner and add a form at the end.

I have included two file formats - .mov and ogv

This needs to be turned to flash, and at the end where it says to "start your free trial", under that it would have "Download Now" and then under that there should be "Email Address:" and then a text input, and then a green button that says "Submit" so that users can submit their email address.

This would need to be delivered in an html page, as the landing page of this submit button would need to be specified in the html. It should not be coded into he flash. Sort of like a clickURL.
