Create Facebook tab

I need to create a simple Facebook tab on my brand FB page to display location of business branches. Currently we have only 2 locations but will have more in future.
We are not able to take advantage of Facebook Locations (as it is only released to selected beta participants) so we need a custom tab to be made. Other than a map with the location, we will also need the details of each outlet to be included (e.g., address, opening hours).
You will need to tell us how to add additional locations in future.
We are looking only for people who have done this before. When you apply, please share examples of other Facebook tabs you have done before. We will not waste time on applicants who do not share any examples and will not discuss our requirements further with you until we see your previous work.
