Clean Up Some Data - Small File

I have a list of Vets names and the state they are in. Unfortunately they aren't in a constant format. Some will have the heading of the state they are in while others will have the state they are in, in the cell row. When you see the attached file you'll understand. I need this Data to follow the following format: First Name, Middle Name, Last Name, City, State That's all I need done. See the attached file
