We would like you to create a flash search form. The parameters in the search form do not need to be connected to anything (our developers will add this). The search form should be simple, but designed nicely, with with a small animation.
## Deliverables
It should be similar in style, but less development needed, to the widgets on:

The widget should be a template, and should be able to be changed easily. You must send a screenshot of the design before.
Successful candidates can get more work for flash and banners.
