We need a module like in:

The module will be developed [login to view URL]
The admin will insert the address of the Physician( country, state, street+ some details . when the end customer will search for a solution he will get the nearest physicians near the entered address this module will be part of .NET CMS
The output of the search will be list of physicians and flags on Google map
We will need an open source and instructions how to use it
