Finish my app to demo

I had a designer working on it but it was too slow.
He was nearly there but had many other commitments and I need it finished for a demo next week.
Its all now simple stuff and will take a few hours for a skilled iOS developer

Why a contest?
1. I want it done quickly
2. I want to find the developer who will come on board long term so I am looking for someone good

The app is running but not complete and need some tweaks. mostly small errors and linking.
I also have the page images that I want copied

Here is the list what to finish (there are also some small grammatical and function points you can spot when you run through it)

1. Opening screen - give two seconds delay on opening screen before going into app

2.a Summary screen to show details from all babies (see attached page graphic)
2.b Appointments page functional and linked with calendar,
2.c Vaccinations page working, list of vaccinations preset, (no need to select)

3.a Edit function working on general information screen for each kids record
3.b Add logo for MIECHV screen and link (see attached)

4. Twitter link to go to open new account

5. Facebook link to open new account.

6. Data is saved in app (still there when you reopen) and add export data as .csv (select file to export or export all)

7. Allergies page minimum, add graphic no links

8. Illnesses page minimum, add graphic no links

9. Growth History* page minimum, add graphic no links (show BMI and BSA on graphic)

10. MIECHV page minimum, add graphic

11. Recording page minimum, add graphic

7/8/9/10/11 not required to function in this contest, just to be there and seen, as these are parts of the development budget once the green light is given. but if you can set them up then its definitely desired as will make the demo even better and your selection more likely

What i want is a demo on my ipad
The demo is just stage 1

I appreciate a contest means you want to do the minimum to impress enough to be selected but if the demo works out then I am looking at a long term relationship and ongoing development work that has a sizable budget ($20,000 or more)
So I have also attached what i want to do and its up to you how far you take it to convince me.

provide skype as well so we can discuss any issues.

  Best thing to do re appts is watch the youtube of the app it follows. The appointment goes into the general info page for the baby and also the summary screen as an upcoming item. When its overdue it goes into an overdue list.
