Linking custom-made questions to MailChimp including automatic e-mail reply

PROJECT DEADLINE 7 JANUARY! NOT READING PROJECTS DESCRIPTION IS NOT GETTING THE PROJECT! --- Dear Freelancers, Currently we are validating our start-ups services, therefore we need your help for the following development: 1. In a couple of days our websites' onepager will go live. This onepager shows all information about our services. Our (potential) customers will fill in their "POSTAL CODE" and their "HOUSE NUMBER" and press the next button. 2. Once they pressed the next button. They will enter a second screen with a pre-designed question. Also this pre-designed question will show the address in a "GOOGLE MAPS" that shows the "POSTAL CODE" and "HOUSE NUMBER" entered before. Below two buttons are situated or go back or next. 3. Once pressed next for the second time, the (potential) customer has to enter "FULL NAME", "EMAIL" and "PHONE NUMBER". Below two buttons are situated or go back or next. 4. Once pressed next for the third time, the (potential) customer has to TICK one of the following BOXES. 5. Same for the following questions. 6. Shows a THANK YOU message. After every step 1,2,3,4 and 5 the possible answers are saved in a MAILCHIMP database. So even when people stop during step 3. Step 1 and 2 are SAVED. Important here is that every (potential) client has a number ID, so every number/ID is a different potential client. Furthermore, once step 6 has succeeded (thank you message). The (potential) client receives an EMAIL with a pre-designed e-mail template with our URL. Once they click this URL they have to fill in addition questions. IMPORTANT is that these following questions will be added to the SAME ID that was used before. So both the first steps 1,2,3,4,5 and the questions filled in after the e-mail reply should be merged to the same ID in MailChimp. All these steps should be linked to GOOGLE ANALYTICS so that we can measure the bounce effect. We already have all these designs.. You as potential freelancer has to use these designs and also make these relatively simple designs RESPONSIVE.
