Create an optin box for a wordpress blog

I’d like to redo the optin box I have in my blog [[login to view URL]][1] and move it a the place of the widget “Watch this first?? and bring the widget “Watch This First?? below as explained in the attachment. IMPORTANT: 1- I'd lik a video design somewhere in the optin box 2- I'd like to see the color to match with the blue in the header graphic. 3- You should show some samples.
