I need a Questionnaires sent to clients (merchants). However, the form needs to include prefilled drop downs and relevant data. Each paragraph contains the choice of data (or values) which will affect our Terms & Rates to be given in Merchant Quote, as well as the type of Merchant Facility and integration we will be able to offer to Merchant. Currently, the Questionnaire is being sent to Merchants offline, as formatted text in the body of email. Our Developer is aiming to get the online version ready as soon as he can, in 2-3 weeks, as it requires a replacement and update of obsolete logics and database inherited from our previous online Instant Merchant Quote. Meanwhile, will be good to find a person who can create an offline Word or PDF input collection form with constructor (Check Boxes, Radio Buttons, Dropdown Menu, Combo Text Boxes, etc.) for Merchant Enrolment Questionnaire.

I have done a similar project for a freelancer client recently. A template form will be created using dropdowns ,checkboxes and textboxes and image for logo. The template will be a PDF which can be downloaded or sent to the client. The client will double click and open the PDF in their desktop. They will fill all the inputs and press SUBMIT or SAVE. When you press submit, the values in the PDF will be transferred to the server where a PHP program will read the values and recreate the PDF filled by the client. SAVE will create a PDF in the local desktop. Mile stone 1 - Create the template PDF using TCPDF to be used by clients Milestone 2 - Code PHP program to recreate the PDF and store on the server with a specific name.
