Import Excel File Data in MySQL Database using PHP

Hello,
we have 6 plain excel files exported from our BMC Remedy ticketing system (in xls and xlsx formats, see attached) and we would like to import them in a mysql database.
The table definition in mysql may be defined in advance, but it is part of the job.
We would like to have a basic PHP page that allows to upload the files, with a form similar to this:
Incident data: Select file (submit button) (checkbox) Delete exisiting data
Change data: Select file (submit button) (checkbox) Delete exisiting data
Problem data: Select file (submit button) (checkbox) Delete exisiting data
Incident to problem data: Select file (submit button) (checkbox) Delete exisiting data
Change to problem data: Select file (submit button) (checkbox) Delete exisiting data
Incident and CI: Select file (submit button) (checkbox) Delete exisiting data
Job has to be developed in coder's environment. Once it works, we will install it in our GNU/Linux Apache + PHP + MySQL server.
