PHP JSON & ARRAYS Pre-Populate a Data Entry Form

Upon page load, I would like a data entry form to pre-populate MULTIPLE rows in each corresponding section from JSON strings. If you click SAVE, it will use existing logic (that works perfectly now) to render the new JSON. I will write these to a database and retrieve them later but for this project I have provided sample JSON strings. Requires PHP, JSON, ARRAY experience. Assuming JSON Decode and Looping through the array to build the form. MUST work properly with included [login to view URL] file to output the new JSON. See [login to view URL] for instructions.
