I run a web hosting service and have lots of experience with malware cleanup. I'll get your Joomla site cleaned up for you. To do the job, I will need server and joomla admin access, along with zips of any plugin/theme you have purchased so I may reinstall. Please note that I will be updating everything, so as long as you have not modified core files, things will go smoothly. Depending on how outdated the install is, updating may cause visual issues/glitches, I can try to resolve these, but for anything complicated you will need a developer/front end person. My focus is on servers, cleanup and security. Cleanup time depends on how large the site is, but Joomla is never fun or short. As far as recommendations, your best and first line of defense is to keep Joomla and plugins up to date. An overwhelming majority of breaches happen due to the main software(joomla/wordpress/magento/prestashop) and/or the modules being out of date. If this is a VPS, I would consider installing Modsecurity along with a firewall, if your server software allow for it. I would get this up as an additional milestone.
