Most of the aspects are clear enouhg in that pdf file (your favorite color it says is blue, by the way), but there's some things that are left to be assumed(listing those below in a sec) and I have some questions as well.
You stress several times that the front-end form fields MUST be pre-populated with the wp user data, which means we should assume the form should be displayed to logged in visitors only. Is this correct? What should happen for non-logged ni visitors(those might not even be registered with the site)?
Related to the above, if the visitor is indeed a registered and logged in user of the site, shouldn't the fields just present the data and not allow editing or if the values are changed that would have to ffect the wp user data?
You say the admin options form should have "- 3 Fields total (2 Text Fields and 1 Pull-down Menu) - 1 Button - Edit, View Shortcode, and Delete Functionality" but did not provide any further insights on what those fields would be and what aspects of teh functionality will they affect, so please forward further information in this regard.
Regarding that shortcode - you did not supply information whether it would have to accept/allow user inputted parameters, or if it would have system-added(hard-coded) ones etc. In this regard you also say "Shortcode Button/Graphic added to the WP toolbar" I suspect you are talking about the post WYSWYG editor toolbar and not the admin toolbar or am I mistaken(please clarify)?
