Convert a small WinForms project into a ASP.NET MVC application. The project is a small medical questionnaire where a patient may enter his name, address, etc. as well as respond to a pre-established list of medical issues. All the code for the portion of the questionnaire already exists in the WinForm application in VB which auto-translates the display English-French based on the contents of an already existing Access table. The ASP project as well as its SQLServer 2008 database (already designed) will be hosted on a popular ASP hosting website. - Create a standard interface to secure login for patients based on usual standard ASP.net pre-established methodologies and on the login information already saved in the SQLServer 2008 database. - Use of standard security methods such as securing code against SQL injections, etc... - Attached are a few snapshots. In all, the questionnaire has a maximum of 4 pages. - The finished ASP.Net project must remain coded in VB.net.

I would propose the delivery of the project in two stages: Creation of views that match the existing windows forms Creation of login system using standard membership providers Supporting components such as data access would be developed with each view/action. I would welcome the opportunity provide a solution and to extract precise acceptance criteria for the project.
