Add simple form to PrestaShop backend

I need a programmer (no agencies, please) to add a simple form module into our PrestaShop backend. We need to add and handle some values in database. Programmer must start working today. Attached screenshot of what we need to add to our PrestaShop. This is the description of each field in this form: First block is called "carrier" We must have a block for each carrier. There may be several carriers for each store in this multistore prestashop. Field: name, type: text Field: phone, type: integer Field: linked_cms page, type: select box containing list of available cms pages Field: linked_store, type: select box containing list of stores in this prestashop Field: status, type: select. Options: offline, waiting, delivering, returning Field: vehicle, type: select, options are scooter and bicycle Fields: idle_time, commute_time, slots, and pending_orders, integers of 2 digits maximum Second form block is called "store" Field: current waiting time, type: integer 2 digits Field: last connectiong, type: linux date, format to show hh:mm:ss. If it is less than 10 seconds ago, it must be green color, else black Field: pending orders, type: number not editable, showing how many pending orders this store has (statuses "accepted" or "delivering") Text field, showing "open" or close", upon current store schedule set up. Fields: delivery times. Type: integer 2 digits. This list must contain opening and closing hours and days. A day can have several opening and closing times. It has to be listed ordered by weekday. It must allow to add more times to table. Fields: max_reply_time y max_statusnew_time, integers of 2 digits maximum Please, answer to this bid with the words "I read your briefing" at the beggining, so we now you are not a bot. Bots will be omitted for good and regular coders protection ;) No agencies, please. All texts in this new module must allow to be translated. You'll be given an SSH and GIT users. Work must be done with GIT control version. Comments and descriptions inside code are appreciated. All input fields must be updated when tab key is pressed. No action button desired, neither to reload page. We need a refresh button to read last values from mysql. Also, we need you to add to order details backend page a new box containing a select box to link a carrier with each new order. This select box must show carrier names available for this store in our multistore prestashop. Thanks.
