PHP based webpages to edit records in a MySQL data table

This project is for the creation of a MySQL data table and webpages that allows a user to view, add, edit and delete records. The SQL table should contain at least the following fields: QuestionID Unique Key TheQuestion Text (255) TypeCode Text(6) (Possible values: SCALE, YES/NO or TEXT) Active Boolean The layout of the webpage should be easy-to-use and intuitive. It should have a grid that allows viewing and scrolling through the data table. The user should be able to easily add, edit and delete entries. The PHP code should be well written with comments and appropriate error handling. The page needs to be able to gracefully handle and report errors to the user. During development, you should be able to provide me with a URL to the working prototype so I can test it and make suggestions. Project Deliverables: The completed project will include SQL scripts to allow me to create the table, indexes, etc; the PHP file that connects to the database and generates the HTML; and CSS file to allow easy customization of the look of the page.
