Need a form created that will download into a google doc database. Must be based on the attached document.
I will need to post a link to the form on my website where the data is entered and then stored in the database.
The document that is attached is a paper form that is currently being used. It appears that Google docs has a spreadsheet that allows a form to be created and the info is downloaded into that spreadsheet in my google account. I need the form to show the results of the formulae illustrated in the directions in the attachement.
If this cannot be done with Google docs, then maybe it can be done in my website which is Php. I use a Joomla 1.5 with Php 5 and MySql 5.
