Changes to Existing Web Site/Forms/DB

Client identified a few issues with the current site 1) Change one field from Radio button to Checkbox to allow them to select multiple options. 2) Add a "Print" button next to the save button to allow the user to print the completed form? The button would pull up the print dialog window. 3) Make it so the completed entries can be searched for AND displayed in a nice looking form that can be printed? (this would be for each of the different forms) 4) Make it possible for them to search for, retrieve and edit a record? 5) Add a counter field called Tracking # to all forms - this would be auto generated? Make the print and display look nice. Make sure that when printed all fields print clean and clearly.
