The user enters a street address, which is autocompleted & verified by Google Maps Place API (I have completed this part of the code)
A URL is built from components of this address
The user clicks GO which calls the URL to a 3rd party server
The 3rd party server returns an XML file
Take the xml response and, in a form on the 2nd page, request more information from the user.
When the user hits submit, validate the input and email it to the site admin
Hide the form <div> and display information based on the contents of the xml file
