Create an Ajax order form

I need someone to create an order form for my phone service.
The order form should be Ajax driven and use the same design as my website.
Source files for the website will be provided.
The order form should have this flow....
Screen 1
Name
Company Name
Address 1
Address 2
Address 3
Postcode
Country Dropdown
Email Address
Discount Code
Screen 2
A different screen (either Screen 2a or 2b) will appear here depending on which ‘Sign Up’ button they clicked on the website
Pricing will be stored in a MySQL table

Price should be updated automatically and discounted if there is a discount code. Codes are stored in a MySQL table. We'll have to work out how best to format these tables.
Screen 2b – Virtual Numbers
Select the required area code
A list of available numbers is generated
The user selects a number
Screen 3
A summary is displayed and then the user submits the order.
The order is sent via email. Virtual numbers are reserved via an Ajax call.
The code should be written in a clear way using english variable names to ensure I can follow the code. This will allow me to adjust/add additional fields where needed.
No CMS is used. The scripts should be written in ASP.
