I have adapted my current (paper) New Client intake form to be able to be completed in the form of an ipad application. I have prepared the format I would like in a MS word document (attached). Once the person completes it and taps "submit" on the last page, it can be saved as a PDF and then emailed to me so I can attach it to their electronic patient file.
