Brilliant Directory form building

I have a website that is hosted through brilliant directories and is built with bootstrap. I need help producing several forms for the site and can get you more specifics.

Project description
Baseballmapz.com is the name of my website. The ultimate vision for my website it to create a directory of every baseball field in the country, in a crowdsourced approach where users can fill out a form that then populates onto a directory profile onto the site. To build off of this user base that is searching for baseball fields, we will also develop a directory of all baseball businesses across the country. Business will be listed to the site in on a paid subscription model ($5 a month). Baseball Trainers can also be listed to the site using similarly to the business model, in a paid fashion ($5 a month). Trainers and businesses alike will have profiles that are created by filling out a form, once the form is complete, it will go to the website administrator for review. Players and families will also be able to be connected to trainers and businesses, that are within 60 miles within the players home.

Revenue Model
The website is focused on creating revenue in three different ways.
· Create a complete directory of both baseball field and businesses across the US, which allows me to sell my traffic to affiliate sites in the baseball industry. (Affiliate Marketing & Advertising)

· Create revenue through a subscription base of trainers and businesses. (Monthly subscription levels)

· Create revenue through the lead generation of connecting prospective trainees to both businesses and trainers. (Lead Gen)

Field Creation Form Description:
When someone submits the Field Creation Form, that information builds out a page that is searchable in the field directory. Outside of major stadiums and parks, baseball fields are often hard to find for example generic high school field A on google maps might be told to be at 123 street, but in reality, this is where the literal high school is. I want to build a directory that is crowd sourced so that way the public can post specific locations for a field or give additional information that can describe the location of the field. When the form is submitted that data creates a page that is searchable in the directory, it also sends an email to the administrator to double check to see if the information is valid or a duplicate.

Business listing Form | Trainer listing Form
The business listing form is for business owners to use to submit their baseball business to added to our directory. Similar to the field creation form, it creates a page that is searchable in the directory and alerts the administrator that a new business is submitted. To post to the business directory one must have a business level subscription which is a paid service. This service works the same for trainers, it is paid service with lead gen capabilities linked to the prospective trainee form. Business and Trainer subscriptions are $5 per month.

Prospective Trainee Form
Users on the site would use the Prospective Trainee Form to be connected with local pitching coaches, hitting coaches, and businesses that offer training programs. These submitted forms would work as leads for the coaches so that way they could train young players. These would deliver to the coaches and businesses in the form of an email and I'd like to limit the leads to 60 miles within the coach or businesses zip code. Leads are sold to businesses and trainers in a non-exclusive fashion for $1 a piece. Importantly when a player notifies which type of training that they are interested in, a lead is sent to coaches, business with corresponding trainers in their area.

There is a lot more detail I can provide but I hope this initial document this provides clarity to what I envision is necessary to launch. I'd love to hear feedback as to whether this is possible using brilliant directories.
