Create a waiver using digital signature on wp website

Looking for someone available right now. I need this done by the end of the day Please check the Mockup to see how the page will look. Process Flow: 1. Member enters his email, name, Barcode ID 2. Member Checks the check boxes “I understand and agree” 3. Member enters his name and date 4. Enter his digital signature and submit 5. After he submits, the signed waiver ( PDF) will be sent to the member’s email and to the admin email. Note the content of waiver should be modifiable. we should be able to modify the text or add a different section from the waiver setting
