PROJECT : DENTAL IMPLANT COST CALCULATOR WEBSITE We require a single page cost calculator website with a subscription form. The cost calculator should have the following : 1. Visitor chooses the country and city where he/she wishes to has his treatment.(each country and city can have different prices). 2. Visitor then chooses type of implant (Different Implant Manufacturing companies have different prices.) type of Dental CROWNS and then selects type of implant treatment required by him. (cost of each treatment shall be given).Patient can choose multiple implant treatments. The total costs will get added. Patient can also choose multiple implants. The cost will get multiplied by no of Implants. 3. Total Cost is calculated and displayed below. 4. On the Right hand side of the above calculator we present a Breakup of the payment in the pie-chart Format. Patient is asked If he is travelling from other place to the said place for treatment? If he/she says NO, then there is no change in the price. we present the prices calculated above in pie chart only If he or she selects- Yes then fixed Airline expenditure of USD 800 is added. Patient is asked to choose the type of hotel accommodation he wants: Budget Hotel, 2 star hotel, 3 star hotel, 4 star hotel Or 5 star hotel. Avg charges of the said type of hotel for 10 days is selected and added to the Pie chart. (we do not require any any hotel selection or exact hotel prices. The prices will be the average for the city) Example of the pie chart describing the cost is mentioned in the link [login to view URL] 5. apply for loan/finance at 0% button is given at bottom. On clicking the said button the subscription form with name, address, phone number, email id, Date of birth, age, Country of residence details are filled and submit button is given. Once submitted the details in subscription form along with calculated costs are submitted to us in email for processing. 6. Find the Implant SPECIALIST button is given right next to apply for loan/finance. On Clicking to the button, the subscription form opens with above said details and on submission the details are passed on to us. 6. Disclaimer is mentioned at the end of the website: we will give the Content for disclaimer. 7. The bottom of the page will have details like Customer support number, Social media page links, Copyright information, blogs(we will redirect the said section to our blogging site), etc.
