Build me a Direct Mail Piece and redesign Business Card

I am an Insurance agency named Built Insurance Brokerage LLC. We branded ourselves for a certain industry we sell liability insurance to as InsureMyRCFE.com. I will be sending a direct mail piece to offer insurance for (RCFE's) Residential Care Facilities for Elderly in CA. We are the leading broker in this industry for the last 2 years. I also took a look at your artwork and recommend having our Graphic Artist give your Letter and Business Card a quick re-design. It might prove helpful to be a little more clear regarding what I want my prospect to do, such as...Sign the document? Mail or Fax it back to you? How I want them to pay? Should they call you or go to your website? Etc. Just seems a little unclear, but maybe I'm not understanding your market. PDF and include bracket field addressing it to the Prospects Name, Address and Facility # somewhere near the top of the designed piece.
